What is required to sign the declaration?

Choose your family doctor, therapist, pediatrician.

Prepare documents:

  • An adult patient or a child 14 years of age and older must bring a Ukrainian passport (ID-card, birth certificate, refugee certificate, residence permit in Ukraine, temporary certificate of a citizen of Ukraine) and a Registration number of the taxpayer's account card (tax ID number).
  • If it is necessary to declare a child under 14 years of age, it is necessary to take the child's birth certificate, his/her TIN, as well as the Ukrainian passport (ID-card, birth certificate, refugee certificate, residence permit in Ukraine, temporary certificate of a citizen of Ukraine) of the legal representative (usually one of the parents).
