We are resuming the conclusion of declarations with the family doctor, general practitioner, pediatrician at the St. Paraskeva Medical Center!
Our doctors work according to the principles of evidence-based medicine and take care of your health.
The advantage is that many specialists have additional specialization. That is, if you have chronic diseases, you can choose a doctor who has more knowledge and experience in this area. For example: in Endocrinology, Cardiology, Pediatric Gastroenterology, immunology, or pulmonology.
What is required to sign the declaration?
Choose your family doctor, therapist, pediatrician.
- An adult patient or a child 14 years of age and older must bring a Ukrainian passport (ID-card, birth certificate, refugee certificate, residence permit in Ukraine, temporary certificate of a citizen of Ukraine) and a Registration number of the taxpayer's account card (tax ID number).
- If it is necessary to declare a child under 14 years of age, it is necessary to take the child's birth certificate, his/her TIN, as well as the Ukrainian passport (ID-card, birth certificate, refugee certificate, residence permit in Ukraine, temporary certificate of a citizen of Ukraine) of the legal representative (usually one of the parents).
